Qualifications/Educational Requirements:


  1. Graduate of an accredited High School or equivalent preferred.

  2. Ability read, write and follow directions.

  3. Must be certified CPR &First Aid

  4. A minimum of one year full-time experience in direct patient in an institutional setting (hospital or nursing facility) or one year full-time experience within the last five years in direct client care in an agency setting.

  5. Work positively and favorably with patients, families, and staff.

  6. Demonstrate compassion, responsibility, and cheerful attitude.

  7. Listed on the nurse aide registry with no finding against the aide relating to client abuse or neglect or misappropriation of client property.

Certified Medication Tech

Service Subitle

The CMT or CMA has to be already certified and Active in the Maryland Board of Nursing and have experience of at least 6 months and also have a Certified Nursing Assistant also active and up to date on the Maryland Board of Nursing.

1. Conduct routine visits to the Client’s place of residence to provide necessary services outlines by the agency and the client’s/ patient.

2. Evaluate the client’s needs thoroughly,

3. Plan and discuss future visits based on a need evaluation covered in later steps

4. Review the level of care and placement instruments

5 Discuss the revised plan with the Nursing Supervisor on a regular basis and keep notes of the changes and progress

6. Prepare progress/ clinical note, review, and initial and date progress note on every client

7. Inform the Registered Nurse, physician, case manager, and other relevant arties of changes in client’s condition

8. Counsel and educate the client and family members on meeting the client’s nursing and related needs

9. Must be able to make choices, decisions and act in the Client's best interest, have the ability to react and remain calm in difficult situations and possess written and verbal skills for effective communication and level of understanding.

Licensed Practical Nurse

Minimum of 2 years experience in a healthcare setting.

Excellent oral and communication skills.

  1. Demonstrate efficient teamwork with the staff.

  2. Demonstrate organizational and time management skills.

  3. Support quality improvement practices. *

  4. Perform nursing procedures according to agency policy and procedures. *

  5. Work under the direction of a RN.

  6. Monitor reactions and patient progress using observation, assessment, and evaluation skills.

  7. Educate patients and family members according to disease process, medications, POC, treatment options, and home care procedures according to the plan of care.*

  8. Report to physician and RN of adverse findings. *

  9. Follow state regulations. *

  10. Coordinate and monitor patient care and services. *

  11. Comply with HIPPA regulations in and out of the office.*

  12. Follow infection control policy in and out of the office.*

  13. Document skilled visit according to guidelines.*

  14. Maintain patient records according to policy and procedures. *

  15. Participate in in-services, workshops, seminars, and self-study courses annually. *
